Afghan Community Health Worker (CHW)
JOB SUMMARY
NEAAO is looking for a full-time community health worker to provide the following services to the Afghan community:
Help patients navigate health care services and other supports to reduce the risk of Covid-19. Network with systems of care to ensure the patient has access to Covid-19 vaccines, testing and treatments.
Keep accurate records of all referrals and interactions, and perform follow-up.
Participate in statewide CHW trainings and regular meetings.
Design outreach tools and explore opportunities to host events that support wellness and encourage proactive health care for all family members.
Build relationships in the community that further health equity and inspire an excellent level of health care for Afghan community members.
Explore and engage with childcare, food, transportation, housing, employment and income, and other resources to improve social supports for community members impacted by health concerns.
Qualifications
Good communication skills in English and Afghan language (Dari preferred)
Commitment to helping people in the community and furthering NEAAO’s mission
Knowledge of New Mainer community, especially those from Afghanistan
Driver’s license and clean driving record
Desire to work with families and individuals in the home and in health care settings
ADDITIONAL INFORMATION
Full time exempt position; state training provided
$20-$25 per hour depending on qualifications
